2013 LINK Spirit of Innovation Award
Home Care & Hospice LINK is excited to announce the launch of the Spirit of Innovation Award.
This award recognizes provider organizations for their innovation, dedication to improvement, development of best practices, and commitment to the success of the home care and hospice industries as demonstrated by a willingness to share these ideas with others.
The finalists will be invited to present a case study of their innovation to their peers and be honored at an award presentation at the Home Care & Hospice LINK Conference in June 2013.
LINK is honored to announce the winners of the inaugural Spirit of Innovation Award. Over 25 providers submitted best-in-class ideas and winners were selected based on the degree of their innovation, impact of their idea, feasibility of application and value to the home care and hospice industry. Winners will present case studies on their unusually successful initiatives at the 2013 LINK event this June.

M.O.D.E.L. Care:
Propelling Change & Outcomes Improvement
Winner: Androscoggin Home Care and Hospice
Androscoggin Home Care & Hospice has developed M.O.D.E.L. Care, a unique operational system design that propels change in culture, clinical practice, support services and partner relationships in a highly reliable way. The impact has been dramatic — consistent improvement in outcomes, patient satisfaction, internal audits and reduced canceled visits.

Integrated Care Management: Patient-Centered, Evidence-Based Care Delivery Coordination
Winner: Sutter Care at Home
Sutter Care at Home's Integrated Care Management (ICM) is a patient-centered, evidence-based care delivery model with care coordinated across providers, settings, and time. By developing this standardized ICM system based on best practices, Sutter has changed both provider and patient behavior to reduce hospitalization rates, 30-day readmissions, and emergent care utilization; and improve HHCAHPS measures and employee engagement.

Patient Health Coaching Program for High Risk Patients
Winner: VNS at St. Francis
VNS at St. Francis' Patient Health Coaching Program combines certified Health Coach RNs with telehealth monitoring and kiosks throughout the service area, dramatically reducing
re-hospitalization rates for high risk patients. The program's success has increased exposure to potential clients and led to a key ACO relationship, establishing a new payer in the VNS system.

Patient-Focused Cost-Effective Care for the Chronically Ill
Winner: CareCycle Solutions
By combining daily patient contact with historical clinical and demographic information, CareCycle Solutions has deployed appropriate interventions for its chronically ill patients. Using a disease acuity score, important changes are highlighted so that clinicians can act promptly, saving the cost of unnecessary visits and reducing re-hospitalizations.

Geriatric Care Management in Senior Living
Winner: Bethesda Health Group
Developed originally as a means to support residents in Bethesda's long term care communities, Bethesda's Home Health Geriatric Care Management program has become so effective it has been expanded to support seniors in other community-based settings. In addition to providing a vital service for seniors, Bethesda helps these communities retain their residents and improve census, resulting in dramatic financial improvements.

Using Smartphone Technology to Foster Comfort, Care & Communications for Hospice Families
Winner: "Hospice of Michigan
Providing real time information and progress reports to geographically distant loved ones is a great challenge for hospice providers. Hospice of Michigan's HOM Cares mobile app provides families with daily updates, and fosters interaction among family members and HOM, enabling them to stay in touch and have access to real-time information anytime, anywhere.
